The standard module evaluation sheet is linked here. The idea is that the evaluation sheet is completed based on judging the module and not any individual lecturer. If any student wishes to comment on the differences between lecturers' approaches to the module, you should do so on the back of the printed form (or end of the electronic one). The information obtained from the feedback is used to ensure that a high standard of teaching is achieved.
